Member Materials

  • ID Cards

    ID cards are typically mailed to members’ homes unless groups specify differently.

    Dental

    Delta Dental PPO Plus Premier
    Dentists will typically need the following information to confirm a member's eligibility:

    • Date of birth
    • Primary member's name
    • Group name

    Members can print a copy of their ID card through Delta Dental of Illinois' Member Connection.

    Members can also download our Delta Dental mobile app to access an electronic version of their ID card, find a dentist, access a cost estimator tool and more. The Delta Dental mobile app is optimized for iOS and Android devices (smart phones and tablets) and is available for free to download in the Apple App Store or Google Play Store.

    DeltaCare

    Members must select a primary DeltaCare dentist. DeltaCare dental facilities receive a monthly roster of DeltaCare patients from Delta Dental of Illinois based on members’ primary dentist selections.

    DeltaCare members can change their primary dentist at any time by calling our customer service department at 800-942-3772. However, all changes must be received prior to the 20th of the month in order for the member's change to be effective for the upcoming month.

    If a member or their dependents make a change after the 20th of the month, the change will take effect in two months.

    If a member needs to see a specialist, their primary dentist must provide a written referral to a DeltaCare network specialist per program guidelines (program guidelines indicate the procedures that can be referred and the protocols associated with such referrals by procedure code). There is an authorization process for both non-emergency and emergency referrals. Members should contact our customer service department at 800-942-3772 to confirm the specialist’s eligibility on any referrals provided.

    DeltaCare ID cards will list the following information:

    • Name
    • Group Number
    • Member ID Number
    • Facility Number

    Vision

    DeltaVision®, in association with the EyeMed Vision Care Access, Select and Insight networks, offers vision care plans that give enrollees/members access to a national network of both independent providers and leading optical retailers. Members and their covered dependents will receive network benefits when they visit a vision care provider in their designated EyeMed network.

    DeltaVision® ID cards are exclusively available in an electronic version. They are not personalized and all that the vision care provider will need to verify coverage is the primary member's name and date of birth. Download the DeltaVision ID card that corresponds with your group's EyeMed Vision Care network below (available in both English and Spanish).

    Members can access their vision benefit information anytime, anywhere on EyeMed's mobile-friendly member website or with EyeMed's free mobile app, which is available to all DeltaVision members. The mobile website and mobile app allow members to access their ID card, view benefit information and find an in-network vision provider, all while on-the-go. To download the EyeMed mobile app, visit the Apple App Store or Google Play Store and search for "EyeMed Members."

    Please note: DeltaVision is provided by ProTec Insurance Company, a wholly owned subsidiary of Delta Dental of Illinois, in association with EyeMed Vision Care networks.

  • Enrollment Kits

    We send a customized enrollment kit specific to your plan. We either send it as a PDF by email to you or bulk mail the quantities requested to you or various HR contacts for distribution to your employees/members. Our preferred method is email, but we work with you to deliver what makes sense for your group.

    The kits include a welcome letter and plan highlight sheet with specifics on your group. Additionally, we include “how to” detailers and other information specific to your plan. Sample detailers are below.

    Enrollment Form

    Our preferred method of accepting enrollment is electronic. Please see our eligibility & enrollment page for more information. If you need a form for new enrollment or change of status, you can use this form. We also have Spanish and Polish versions.

    Member Connection

    Members can review their dental benefits through the Member Connection.

    Enhanced Benefits Program (if applicable)

    Delta Dental of Illinois offers an Enhanced Benefit Program that enhances coverage for individuals who have specific health conditions or special needs that can be positively affected by additional oral health care. These enhancements are based on scientific evidence that shows treating and preventing oral disease in these situations can improve overall health. If a member is eligible, they can sign up through the Member Connection.

    DeltaVision (if applicable)

    Delta Dental of Illinois offers a vision plan as well. DeltaVision, in association with the EyeMed Vision Care Access, Select and Insight networks, offers you access to a national network of both independent providers and leading optical retailers. Finding a network vision provider is easy with DeltaVision.

    DeltaCare®(if applicable)

    We also offer a dental HMO option, DeltaCare.

    Help members better understand their oral health with a simple risk assessment tool.

    To GoSM (if applicable)

    The To Go feature – an option for Delta Dental PPOTM* and Delta Dental Premier® programs – ensures that enrollees don’t have to leave unused annual maximums behind. With Delta Dental of Illinois’ To Go feature, members can take the unused amount “to go” from one year to the next**. This option offers members more flexibility and can help them prepare for more extensive and costly dental treatment. Learn more about this feature.

    * The To Go feature is not available for Delta Dental PPO – MAC and MAC Voluntary Plans. Contact your sales executive for complete underwriting guidelines; this feature may not be available for all Delta Dental PPO and Delta Dental Premier plans.
    ** Members cannot take unused maximums with them upon termination of employment or the dental plan, nor can they apply the unused annual maximum to another dental plan. To Go is a trademark of Delta Dental of Illinois.
